These awards aim to promote Belgian initiatives that significantly ameliorate the journey of patients with heart failure.

It rewards three projects that significantly improve either the early detection and diagnosis of heart failure, the clinical implementation of guideline-directed therapy or the collaboration between specialized and primary care, awarding a prize of € 3,000 per winning project.

The applications will be evaluated by an independent jury. The winning projects will be presented at the AstraZeneca symposium “Advancing Heart Failure Care: Belgian Initiatives for Patient Journey Improvement” during the congress organized by the Belgian Society of Cardiology on February 6^th, 2025.
